In partnership with Pharmacy 575, Hobbs Schools will be offering the annual flu vaccine to students at no cost, administered by a team of HMS school nurses. If you would like your child to receive the flu vaccine, please fill out the consent form completely and sign. You are required to complete all information at the top of the form, including the insurance information, medical screening questions and your signature. Please return the completed and signed printed form to the HHS school nurse by November 19, 2025. A turn in box is available in the school nurses office for submission of the consent forms.
Paper consent forms will be available at the High School Office if you cannot access or print one. THESE VACCINES ARE NOT REQUIRED. They are being offered as a service to our students.

En colaboración con la Farmacia 575, las Escuelas Hobbs ofrecerán la vacuna anual contra la gripe a los estudiantes sin costo, administrada por un equipo de enfermeras escolares de HMS. Si desea que su hijo/a reciba la vacuna contra la gripe, complete el formulario de consentimiento y fírmelo. Debe completar toda la información en la parte superior del formulario, incluyendo la información del seguro, las preguntas de evaluación médica y su firma. Devuelva el formulario impreso, completo y firmado, a la enfermera escolar de HHS antes del 19 de noviembre de 2025. Hay una caja de entrega disponible en la enfermería escolar para entregar los formularios de consentimiento.
Los formularios de consentimiento impresos estarán disponibles en la oficina de la escuela secundaria si no puede acceder a uno o imprimirlo. ESTAS VACUNAS NO SON OBLIGATORIAS. Se ofrecen como un servicio a nuestros estudiantes.
